CIS Notice of Appointment of Interview for Adjustment of Status to that of a Permanent Resident, Chicago, Illinois

Please appear at the CIS Office, 230 South Dearborn Street, 2nd Floor,
Chicago, IL on ________ at ______ AM/PM.

You and your petitioner must bring photo identification. If your
petitioner is a Permanent Resident, he or she must bring their Alien
Registration Card. Bring original Advance Parole Document and EAD, if applicable;

Failure to appear for the scheduled interview will be deemed an
abandonment of the application and will result in a denial of the
application pursuant to 8 CFR 103.2(b)(13). Also, failure to bring the
documents requested of you as indicated on the attached "Adjustment
Interview Documentary Requirements" may result in denial of the
application due to lack of prosecution.

Please read this BCIS appointment notice carefully and follow the
instructions below:

Both of you must appear at the interview; You must bring evidence of
your legal entry into the United States including your passport and
original form I-94. We need to submit an updated 1-864 Affidavit of
Support and last three years of tax returns, W2s and a current job
letter from the Petitioner; Please call, 312- 836- 9040 or in the

FOR ALL APPLICATIONS:

1. All foreign document(s) must be accompanied with English translation at the interview;

2. Bring original proof of previously approved I-130, I-140.or consular letter, if relevant; (Original Form I-797, Notice of Action from CIS)

MARRIAGE BASED APPLICANTS

1. Official Birth certificates for you any your spouse and any children,
yours and your spouse's, born in or outside the United States along with
any English translations.

2. Proof of Petitoner's status, such as birth certificate,
Naturalization certificate, US Passport, Alien Registration Card;

3. Official marriage certificate for Petitioner and beneficiary and
proof of termination of previous marriage, such as divorce divorce
decrees and death certificates;

4. Wedding photos and other photos of you and your spouse taken during
your entire relationship;

5. Proof of any joint purchases or ownership, such as TV, furniture,
car, house, etc;

6. Proof of joint credit established since marriage, such as joint
credit cards, loans, etc;

7. Proof of actual residence such as leases agreements, property deeds
or mortgages, etc;

8. Proof of bank statements for checking and/or savings accounts and
canceled checks throughout your marriage;

9. Proof of medical, life and auto insurance during your marriage;

10. Proof of utility bills for a shared residence, such as telephone,
electric, gas, etc, for the duration of your marriage;

11. Federal income tax returns with W2 forms for the last three years,
along with current employment letter; W 2's last 3 months;

COURT DISPOSITIONS (divorce Judgments, etc.):

AFFIDAVIT OF SUPPORT (form I-864): Must be updated and submitted with current employment verification/job letter for sponsor and federal taxes last 3 years w/ W-2's for 3 months prior to interview;

MEDICAL EXAMINATION (form I-693) and immunization supplement: LIST OF DESIGNATED CIVIL SURGEONS, CHICAGO: The medical examination must be conducted by a civil surgeon who has been designated by the BCIS. Go to: http://www.immigration.gov/graphics/fieldoffices/chicago/CivilSurgeonList.htm#chicago
